1.  Please write the check to:  Greene County Community Foundation.  If it is for a specific fund other than the permanent endowment, indicate the name of the fund on the memo line (i.e. Greene County 4-H is a fund within GCCF).

2.  Please include your NAME, ADDRESS, PHONE and whether or not you wish to receive Endow Iowa Tax Credits for your contribution.

3.  Please indicate whether we may use your name as a donor in the media or on our website.
